Download the Release and Waiver (MS Word). Read the Document Information and Instructions cover page carefully, making sure to complete the word-fillable fields in the Introductory Paragraph and Signature Block of the document.

Fill in the following fields in the Introductory Paragraph:

• Official Program Name (please consult your acceptance or nomination letter for the complete and accurate name of your program)
• Campus: fill in as the Twin Cities campus
• Term of your program

Fill in the following fields in the Signature Block:

• Your name (type or print clearly)
• Your date of birth
• Signature

If you do not fill in the appropriate information on the Release and Waiver, you will be required to resubmit it.

Please take note that by signing this form, you indicate that you have read and accepted the responsibilities mentioned in the Release and Waiver.

You are responsible for the information contained in the Learning Abroad Center's Cancellation Policy.

The learning abroad activity in which you plan to participate is subject to the University of Minnesota Policy on Suspending Education Abroad Opportunities due to Health and Safety Risks.

While participating in this program, you are subject to the University of Minnesota Student Conduct Code.
