Step by Step Instructions to Fill Out the Visa Application
Clearly write "Student Visa" on the top of the application.
- Last name.
- If you have changed your last name
(through marriage for example), former last name.
- First and middle names.
- Date of birth (year-month-day).
- Leave this blank.
- Country where you were born.
- Your nationality. (If you were born
in the US, please put US)
- Your previous nationality, if you
have not always been a US citizen.
- Gender.
- Marital status.
- Father's first name.
- Mother's first name.
- Type of Passport. Circle NATIONAL
PASSPORT, unless you have a different type of passport.
- Passport number.
- Enter US Department of State or the passport agency that issued your passport (NOT where you applied).
- Date your passport was issued (listed in your passport).
- Date that your passport expires (must be at least one year from the date the visa application will be submitted to the consulate).
- Write N/A, unless you are not from
the US but reside in the US.
- Write STUDENT.
- Write University of Minnesota,
230 Heller Hall, 271 19th Ave S, Minneapolis, MN 55455.
- Write Toledo, Spain.
- Semester students circle LONG STAY.
Academic Year students circle SHORT STAY; you will be applying
for a longer visa once you are in Spain.
- Circle INDIVIDUAL.
- Circle MULTIPLE ENTRIES.
- Calculate the number of days between
your arrival date and departure date.
- If applicable, specify previous dates and
places where visas were granted.
- Write N/A.
- If applicable, Schengen states that
you have visited and corresponding dates. (These countries are
listed on the consulate's web site.)
- Circle OTHER and write STUDY.
- Write the date of your arrival (i.e.
9/7/05). You may estimate your arrival and departure dates if you have not purchased a flight at the time of application for the visa. It is better to estimate earlier for your arrival and later for your departure. For semester students, the total number of days should be less than 180.
- Write the date of your departure
(i.e. 12/14/05). Remember to include travel time after the program.
- Write the first entry city in Europe.
- Write AIRPLANE.

you plan to use. Be sure to circle HEALTH INSURANCE as you will receive CISI international health insurance. (The date of insurance coverage should be the same as or later than your departure date. If you plan to travel after the program ends, you should purchase additional coverage. Your CISI coverage can be extended for one month. If you are staying longer than one month, you should purchase additional coverage on your own.)
